Triggering different emails based on answers
The second question of my form are what compagnie do you choose , with 19 differents compagnie, how can i attach a email adress to each one of the 19 differents compagnie so the will received that form when it’s filled

It’s Salim here at forms.app. It seems you like to trigger different email notifications based on answers. This is not possible at the moment; however, it is a really brilliant suggestion. I’ve adjusted the title for this, so it gets more recognition and support.
In the meantime, you can create a custom workflow on Zapier by connecting the following apps:
forms.app as the trigger
Filter by Zapier to redirect based on the answer to your second question
Your preferred email app to send emails
