Custom Contact Fields for Questionnaires

The current contact forms are only capable of collecting respondents’ name, address, email, and phone number. However, in a questionnaire, many other types of data may be required. For example, company name, distinguishing between a personal mobile number and a company phone number, or job title. It is also disruptive that each data point appears on a separate page. These could instead be collected within a single box, and questionnaire creators could configure which data fields they want to request.

    Hey Ágnes,

It’s Salim here at forms.app. Thank you for taking the time to share your ideas.

I’ll make sure to bring our product team’s attention to your request: a custom contact field in a single block.

In the meantime, you can follow these steps to achieve similar results:

    1. Switch to list-view by opening your form > design tab > layout:

    2. Add a page break to your form by dragging and dropping it from the fields menu. This will make sure all your contact fields are shown on a single page:

    3. Add an explanation field above each page and use the title format for organizing your pages. This way, your audience will clearly see the structure of your form.

That’s it! For Company name and other details, it is possible to use the “Short Text“, “Phone“, and “Email“ form fields.

I hope this is helpful. If you have further questions, we’d be glad to assist you here or through live support. Thank you again for taking the time to share your ideas! 🌿

      Dear Salim,
thank you for taking the time to look into my suggestion.
It is indeed technically feasible this way, and it works as an emergency solution, but from a UX/UI perspective it is quite inconvenient for the respondent, as each new question takes up too much space, and filling it out this way is also more tiring than completing it in a single block. Seeing a complete form makes it easier for users to fill it out than having new data-related questions appear continuously, again and again.

From a development perspective, this is probably not a huge effort, but it would be an improvement for both the questionnaire creators and the respondents.
